Expanded bug-report issue template

Motivation & Changes:

• Some users don't know how to get logs - added links to the HA documentation.
• Further added additional notes for provide integration debug logs, and basic info on the system logs.
• Added step to provide graphs, for debugging control loop / calibration issues
• Regrouped items a bit, removed Prerequisites (redundant with all the info below now)
